– Appointment procedure –

• Step1: Parents/Guardians are required to fill in the play therapy [appointment form].
• Step 2: Please kindly wait for the confirmation message from Admin via Whatsapp.
• Step 3: Once the appointment is successfully arranged, please make the payment in the shopping cart and choose the number of session based on the confirmation message.

*[Note]: No refund will be given for those who make the payment without receiving the confirmation from admin department.

– Notice for Play Therapy –

• 1. The parent/guardian needs the child’s consent to arrange the play therapy session.
• 2. Play therapy is suitable for chiled who are 3 to 12 years old, and each session is carried out in one-on-one form for about 40 minutes per session.
• 3. Each session would be charged for RM30 maintenance fee, while we would collect 6 sessions: fee RM180 (non-refundable) once have the play therapy time arranged. Volunteers will discuss the child’s progress with parents before a treatment ends to make plans for the next step.
• 4. Admin would arrange your session according to waiting list and available time slot. It is estimated to take around 3 months for successful arrangement.
• 5. Life Line cannot guarantee to meet specific expectations of guardians, nor promise to cure or directly reduce the symptoms of special children.
