Terms & Conditions \u6761\u89c4 (Recurring Donation)<\/h3>\n

Auto Debit Selection of Debit\/Credit Card Authorization for Recurring Donation:<\/strong><\/p>\n

1. The Recurring Donation is only available for credit\/debit card payment.<\/p>\n

2. Card transaction is processed every 25th of the month, successful transaction will be notified by Life Line Association Malaysia (LLAM) via email within 3 days after each transaction<\/p>\n

3. The recurring transaction will stay in effect according the donation period that I have filled in (E.g: if I choose to donate for one year, Life Line Association Malaysia (LLAM) will inform about this service one year later).<\/p>\n

4. If I choose to terminate this service, I agree to give a 15 days written notice or by submitting updated information.<\/p>\n

5. I have confirmed the personal information above is correct and will be stated in the receipt issued.<\/p>\n

6. All personal data will be protected, and donation will be proceeded by Life Line Association Malaysia (LLAM). I have refer to Privacy Policy and Donation Refund Policy for more details.<\/p>\n

\u5b9a\u671f\u5c0f\u989d\u4e50\u6350\u4fe1\u7528\u5361\u81ea\u52a8\u8f6c\u8d26\u6761\u89c4 :<\/strong><\/p>\n

1. \u6b64\u5b9a\u671f\u5c0f\u989d\u4e50\u6350\u4ec5\u9650\u4e8e\u4fe1\u7528\u5361\u81ea\u52a8\u8f6c\u8d26\u3002<\/p>\n

2. \u6b64\u670d\u52a1\u5c06\u5728\u6bcf\u6708\u7684 25 \u53f7\u8fdb\u884c\u6b3e\u9879\u6263\u9664\uff0c\u751f\u547d\u7ebf\u534f\u4f1a\u884c\u653f\u5904\u5c06\u4f1a\u5728\u6bcf\u6b21\u7684\u81ea\u52a8\u8f6c\u8d26\u670d\u52a1\u901a\u8fc7\u540e\u7684 3 \u5929\u5185\u900f\u8fc7\u90ae\u4ef6\u7ed9\u4e88\u901a\u77e5\u3002<\/p>\n

3. \u6b64\u5b9a\u671f\u4e50\u6350\u5c06\u4f1a\u6709\u6548\u81f3\u672c\u4eba\u5728\u8868\u683c\u4e0a\u586b\u5199\u7684\u4e50\u6350\u671f\u9650\uff08\u4f8b\u5982\uff1a\u672c\u4eba\u5df2\u9009\u62e9\u4e50\u6350\u4e00\u5e74\uff0c\u884c\u653f\u5904\u5c06\u4f1a\u5728\u4e00\u5e74\u540e\u544a\u77e5\u6b64\u9879\u4e50\u6350\u670d\u52a1\uff09\u3002<\/p>\n

4. \u82e5\u672c\u4eba\u6b32\u7ed3\u675f\u6b64\u4e50\u6350\u670d\u52a1\uff0c\u672c\u4eba\u540c\u610f\u901a\u77e5\u884c\u653f\u5904\u5e76\u7ed9\u4e88 15 \u5929\u7684\u65f6\u95f4\u5904\u7406\u3002<\/p>\n

5. \u672c\u4eba\u786e\u8ba4\u4ee5\u4e0a\u7684\u6350\u6b3e\u8d44\u6599\u65e0\u8bef\uff0c\u5e76\u4ee5\u6b64\u6ce8\u660e\u4e8e\u6536\u636e\u4e0a\u3002<\/p>\n
